It was designed and constructed by Polish-born Ralph Modjeski, and the cables for the bridge were designed by Leon Moisseiff, who later designed the Tacoma Strait Suspension Bridge that collapsed in 1940. It was the third of three bridges spanning the East River, following the Brooklyn Bridge (1883) and the Williamsburg Bridge (1903).

The Manhattan Bridge is designed as a two-level road, with two-way 4-lane roads on the upper level and two-way convertible 3-lane roads on the lower level. The 3 lanes can all be turned into one-way traffic during peak traffic hours to ease traffic congestion. In addition to cars, the bridge carries four subway lines, a pedestrian lane, and a separate bikeway. The protagonist is a baroque stone arch, inspired by the Porte Saint-Denis, the gateway to Paris, which forms the access to the bridge from the direction of Manhattan.
